GERIATRIC CARE
Dr. Faro T. Owiesy at CORONA DOCTORS MEDICAL CLINICS, INC. has extensive experience providing geriatric care in the hospice or in-home visiting setting. He is dedicating to preserving the health and lives of seniors whom reside in nursing homes, assisted living facilities, and board and care homes. He provides an exceptionally high quality of personalized care for his patients. Dr. Owiesy is a multi-specialty trained physician who uses the “home-visiting physician concept” to improve the overall health of patients, resulting in fewer hospital admissions.

In-Home Visiting Physician

Per media the current state of the sickest elderly citizens of the United States is dysfunctional. Why 5% of Medicare beneficiaries account for more than 50% of medical expenditures? It is difficult but necessary to admit the harsh reality that nobody really wants to take care of sickest aged patients because of time consumption, complexity of condition, and lack of reimbursement. These senior patients are abandoned and neglected, at great financial and personal cost.  Their very neglect results in the ultimate costs. Those to whom the care of the sickest elderly patients defaults are unable to provide care in an effective manner. Visiting nurses are clinically and legally unequipped to handle the care of the sickest 5% of senior patients. Racial, ethnic and socioeconomic factors add to the problems of timely access and intervention. Another cause is the no-coverage of many prescription drugs by Medicare  part  D. Lack of access to correct medications at the correct times adds to the overall costs of care. Medical malpractice costs rise because of lawsuits, causing physicians to practice defensively and limit their liability by automatically sending frail elderly  to the hospital. The solution lies in better management and earlier intervention provided by IN- HOME physician concept.

Dedication to the concept of Home, Nursing Home and Assisted Living visiting physician could help halt the cycle of sick elderly patients bouncing from Emergency Room to Hospital to Nursing Home (Skilled Nursing Facility), and back again, unnecessary consuming billions in health care resources. Home  Visiting Physicians, using the tools of medicine and communication, providing quality of care and support to our burgeoning elderly population and  disabled and home bounded adult struggling to assist them.

Senior patients admitted to custodial nursing care facilities such as Assisted Living and Board and Care facilities tend to frailer and have reduced functional capabilities than those who can remain in the home.  In the home visiting physician delivery system our patients are typically in their homes.

The home visiting physician system would avoid and reduce the neglect and lack of professional interests in care of chronically ill elderly such as:

  • Access issues and neglect:  lack of timely access to physician leads to clinical deterioration, increase hospital utilization and poor outcomes.
  • Health care of the sickest patients is uncoordinated and disjointed because of the ill-timed involvement of a confused and confusing plethora of physicians, institutions and caregivers.
  • Dissatisfaction of patient and family due to failure of meeting their expectations
  • Lack of education of sick patients, and their families, medication costs, non-compliance with medical and dietary advice.
  • Unmanaged prescription benefits that should be managed in the patient’s residence by a physician.
  • Socioeconomic issues lead to medical deterioration, acute hospitalizations, and poor quality of life.
  • Hospital emergency rooms as an alternative are expensive, overcrowded and do not able to address long term care issues.
  • Multiple prescribing physicians lead to redundancy, increases costs, and increased risk of adverse drug/ or treatment interactions.

The newest trend in the care of increasing number of frail and chronically ill elderly in the United States, may lead in the next future to appropriate levels of care and avoidance of inappropriate admissions and costs. In addition, it may restore dignity to fragile elderly, especially the incurably sick, via strengthening the relationship between doctors and patients’ families.

We are dedicated to chronic care of frail elderly patients at their Home, Assisted Living, and Board and Care facilities. For more than 6 years, Corona Doctors Medical Clinics has provided services of “In home physician visit” to chronically ill patients. We believe a better way of delivering health care to home bounded elderly patients, is returning to old fashioned idea: the doctor house call.

We provide services 24/7 access. We provide a system of cooperation with several home health agency-nurses as   home nurse visit, physical therapists, skilled nursing facilities, pharmacy delivery system, and Hospice Care (end of the life and palliative care) agency-nurses as well as   hospitalist physicians. This system  optimizing the patient’s health status in home helps avoid unnecessary Emergency Room (ER)  visits and hospitalizations, reduces the patient’s direct health care costs including deductibles, co-insurance and other fee-for-service expenses related to hospitalization.
